Preparing Youth for Success serves Erie County youth who are referred from Erie County Probation or Detention. The program supports youth in accomplishing their goals through a combination of case management, youth mentoring and vocational training services.
About this Program
Hillside’s Preparing Youth for Success program serves Erie County youth who are referred from Erie County Probation or Detention. This program offers three components to support youth in accomplishing their goals:
Case Management: The Care Coordinator serves as a life coach to help encourage youth as they navigate through this pivotal life transition. The relationship and connection made with the Care Coordinator will further strengthen the youth’s participation in outside activities while creating comprehensive wraparound services to support their transition back into the community. The Care Coordinator will link youth in this program to community resources that support the accomplishment of their goals and transition into the community.
Youth Mentoring: The Youth Peer Mentor provides formal and informal supports to youth in the program. This service supports engagement and active participation of the youth in their treatment planning process. Activities are intended to develop and achieve the identified goals and objectives as outline in the youth’s treatment plan.
Vocational Training: Hillside provides vocational training using the Columbia University Workplace Center’s Young Adult Work Opportunities for Rewarding Careers (YA WORC) model. This evidence-informed model is developmentally appropriate for adolescents and was designed for youth who have experienced Juvenile Justice, Child Welfare, and/or Foster Care system involvement.
