Qualified Residential Treatment Program (QRTP) Aftercare

At Hillside, we understand that residential treatment is only one step in the care that a vulnerable youth may need. Shortly after a youth is admitted to a Qualified Residential Treatment Program (QRTP) on a Hillside campus, our staff begin the process of preparing the youth and their family for the next steps that will follow residential treatment. QRTP Aftercare provides ongoing services for youth and families to help them continue their path to healing.

About this Program

QRTP Aftercare provides discharge planning beginning within 72 hours of the youth’s admission into the program.  Goals related to discharge and aftercare are developed and presented in the first treatment plan and in every subsequent treatment plan. The goals include clear and measurable objectives which may change during the youth’s time in the QRTP to reflect progress toward achieving the goals.

Once a youth is discharged from the QRTP, the Aftercare services they receive will include the following:

  • Casework contacts with the youth, which will consist of at least two face-to-face contacts during the first month following their discharge from residential treatment, and at least once per month thereafter. These contacts will occur in the location where the youth resides at least once within the first post-discharge month, and at least once every 60 days thereafter.
  • Casework contacts with the youth’s family or permanency resource will also be conducted. These will consist of at least two face-to-face contacts during the first month following the youth’s discharge from residential treatment, and at least once per month thereafter. The contacts will occur in the location where the youth resides at least once within the first post-discharge month, and at least once every 60 days thereafter.
  • For youth 18 years or older, face-to-face contacts may be conducted via video call if the youth and the local Department of Social Services agree to such an arrangement and the youth has the necessary technology to allow for such communications.
  • Meetings with the permanency team, including the family, no less than twice during the aftercare period, including once no less than 30 days prior to cessation of aftercare activities.
  • Comprehensive aftercare services identified in the last treatment plan prepared by the QRTP.

The length of QRTP Aftercare will vary depending on the needs of the youth, but will often last approximately six months following discharge from residential treatment.

Eligibility

This program is provided to youth who are admitted to a Hillside Qualified Residential Treatment Program (QRTP).

The program serves New York State counties including Allegany, Broome, Cattaraugus, Chemung, Chenango, Cortland, Erie, Genesee, Herkimer, Jefferson, Lewis, Livingston, Madison, Monroe, Niagara, Oneida, Onondaga, Ontario, Orleans, Oswego, Schuyler, Seneca, Steuben, Tioga, Tomkins, Wayne, Wyoming and Yates. (Hillside may accept referrals on a case-by-case basis from these counties: Albany, Clinton, Columbia, Delaware, Essex, Franklin, Fulton, Greene, Hamilton, Montgomery, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, St. Lawrence, Warren and Washington.
